This is done by using DDE macros with Program Manager.
To learn more about it, check out "Shell Dynamic Data-Exchange Interface
Overview" in Delphi's Help.

Ready made answer can be found from Delphi Super Page.
The name of the program is something like COOLDDE or
DDECOOL, and full source included.
